Project: To improve the percentage of patients seen for an appointment at AF Williams Clinic within 14 days of Family Medicine Service Inpatient Discharge from 57% to 85% by January 1, 2016.
Project: To open the new AMC Burn Outpatient clinic by 6/26/15. The second project AIM is to treat 50 patients per week in the new outpatient clinic by 8/31/15.
Project: To create treatment plans for an average of 4 patients per day who develop acute care related symptoms in an area other than the ED or another clinic.
Project: To improve population management for children/families that return to clinic rarely after their initial visit.
