Project: To improve the percentage of patients seen for an appointment at AF Williams Clinic within 14 days of Family Medicine Service Inpatient Discharge from 57% to 85% by January 1, 2016.

Barbara Davis Center Team

Project: We will increase the capture rate of all screening types (eye, TSH, lipids) by 10% by October of 2015.
 

Burn Center Team

Project: To open the new AMC Burn Outpatient clinic by 6/26/15. The second project AIM is to treat 50 patients per week in the new outpatient clinic by 8/31/15.

Cancer Center Infusion Center Team
