Dear Colleagues,
We are pleased to bring you the latest Clinically Speaking Newsletter, this one focused on an area of growing importance in the world of medicine.
In this issue, we feature the many ways in which the CU School of Medicine is engaging in transitional care for patients with conditions that continue into adulthood – including the Improving Pediatric to Adult Care Transition (ImPACT) Program, which aims to successfully transition those patients to adult care; a new referral process between Children’s Hospital Colorado and UCHealth that helps streamline and standardize transition of care; the Adult Special Care Clinic at Children’s Colorado; another new initiative focused on inpatient care; and transitional care teams in specialties including neurology, cardiology, and urology.
As Daniel Wood, MD, PhD, who is building a transitional urology team at Children’s Colorado and UCHealth, puts it: “For patients and parents, the security of knowing there is somebody who understands the complexity of their diagnosis, who understands the treatment they've had, and is part of a team that's capable of managing the ongoing consequences of both of those is really important. There are data out there that show us that if transitional care isn't there, the long-term health care outcomes aren’t as good.”
