Strategic Plan Five Key Aims

We will achieve our Vision through five Key Aims:

I. Patient Care:
Provide outstanding Family Medicine to every patient.

II. Research: Create and disseminate new knowledge.

III. Training: Train a future integrated, Family Medicine workforce.

IV. Community Engagement: Achieve health equity with community engagement and advocacy.

V. Our People: Nurture a thriving, diverse and equitable department culture.

You'll notice in the graphic that 'our people' are intentionally at the center of all of our Key Aims as they are integral to all that we do and how we will achieve these Aims. Note that the numbered order of the Key Aims and our Goals (listed below) does not reflect a prioritization or sequencing of our activities to achieve them.

Strategic Plan Guiding Principles 2024

And we will adhere to the following Guiding Principles:

Innovation:
We embrace creative and progressive approaches, fueled by curiosity, to advance health and well-being.

Collaboration: We foster effective partnerships and teamwork that enhance interdisciplinary approaches, knowledge sharing, and our overall effectiveness.

Compassion: We demonstrate empathy, kindness, and patient-centered care that prioritizes the health, emotional and social well-being of all.

Advocacy: We passionately promote health and well-being by championing the needs of patients and the community.

Integrity: We uphold the highest ethical standards in all interactions, ensuring trust and transparency in everything we do.

Diversity, Equity and Inclusion: We place the highest value on diversity, equity, inclusion and anti-racism, creating an inclusive and just environment for all.

This wheel, entitled the Circle of Excellence, signifies that we continually strive for excellence in adhering to our Guiding Principles, and in all that we do.

Our Goals for each of the Five Aims are listed below. Each goal has a series of tactics and metrics which will be used to track our efforts, efficacy and impact. As we begin the implementation process, we will introduce an accountability tracker for each area which will be regularly updated so that we are transparent in our progress toward achieving our Goals.
