Summary

Train the Trainer (TTT) is focused on developing the knowledge, confidence, and skills to support engagement in ACC 3.0 and other value-based payment opportunities to Practice Facilitators and Regional Accountable Entity (RAE) liaisons across Colorado. Throughout the trainings, Practice Facilitators and RAE's will gain the skills to support changes in culture and care delivery models, along with administrative skills, including data management and budgeting to enable practices to succeed and thrive in value based payment.

Significance

Supporting providers, practice facilitators (PF’s), and Regional Accountable Entities (RAE’s) in Colorado to engage in  Accountable Care Collaborative Phase III (ACC 3.0) and other Value Based Payments (VBP) opportunities. Shifting Primary Care payment from Fee-For-Service to VBP will help improve quality of care and increase investment in Primary Care. 

 

Impact

The impact of this project will ultimately help patients with quality of care, close health disparities, improve health care access, and help manage healthcare costs.
