The RSS requires that retrospective chart reviews be approved through their application and review process. One copy of each of the following documents must be submitted:

• RSS Application
• Final Protocol - if applicable
• COMIRB application - InfoEd applications do not require signature
• COMIRB Certificate of Exemption - or your letter of approval for Expedited studies
• HIPAA Authorization - either Form B, A, or a certificate of HIPAA waiver
• Budget - if the study is unfunded, include a statement indicating that there is no budget
• Data Request Form

If your prospective clinical trial is being carried out at University of Colorado Hospital, you will need to submit the following documents to the RSS for review:

• RSS application
• Protocol
• COMIRB application
• COMIRB approval letter
• Informed Consent Form
• HIPPA Authorization B, and A (if applicable)
• Study Budget
• Clinical Trial Agreement with Statement of Work and Letter of Indemnification
