UCHealth Research Administration (RA)
Submitting to the RA
The UCHealth Research Administration (RA), provides oversight for any research study using UCH facilities medical records. This
includes exempt protocols and retrospective chart review.

The UCHealth Research Administration requires that retrospective chart reviews be approved through their application and review process. One copy of each of the following documents must be submitted:
- UCHealth Research Administration Application
- Final Protocol - if applicable
- COMIRB application - InfoEd applications do not require signature
- COMIRB Certificate of Exemption - or your letter of approval for Expedited studies
- HIPAA Authorization - either Form B, A, or a certificate of HIPAA waiver
- Budget - if the study is unfunded, include a statement indicating that there is no budget
- Data Request Form

If your prospective clinical trial is being carried out at University of Colorado Hospital, you will need to submit the following documents to the UCHealth Research Administration for review:
- UCHealth Research Administration application
- Protocol
- COMIRB application
- COMIRB approval letter
- Informed Consent Form
- HIPPA Authorization B, and A (if applicable)
- Study Budget
- Clinical Trial Agreement with Statement of Work and Letter of Indemnification
Completed applications can be sent to Kim Dimmit at campus box F417 or dropped off on the 10th floor of Leprino. All forms can be accessed on the next tab.
