New Patients 

Welcome to the HTC! As a new patient, or caregiver to a new patient, it can be overwhelming to remember all you need for your first visits. 

To prepare for your appointment, please keep the following in mind:

• Speak with our front desk about anything you may need to bring as some of our office protocols may have changed
• Obtain an insurance authorization/referral as needed

Remember to bring: 

• Health Insurance card(s)
• Infusion logs, if applicable
• Pertinent medical records
• Names, addresses & phone numbers of your Primary Care Physician and Dentist
• Wear or bring a pair of shorts for physical therapy examination
• Pediatric Patients: Immunization Records
