The Connect to Care Program provides patient navigation assistance to women diagnosed with cancer through the Womens’ Wellness Connection to get treatment.
WWC is Colorado’s statewide program to provide free breast and cervical cancer screening for women who qualify. WWC is funded by the Centers for Disease Control and managed by the Colorado Department of Public Health and Environment. The WWC program is available at more than 100 health care providers across Colorado. Women who are screened through WWC and found to have breast or cervical cancer, may qualify for treatment through the Breast and Cervical Cancer Program (BCCP). BCCP is a Medicaid program for patients who have been diagnosed with breast or cervical cancer who meet qualification criteria.
Women who are screened and diagnosed with breast or cervical cancer through WWC who do not qualify for BCCP Medicaid, are eligible to receive navigation assistance through Connect to Care.
COE is partnering with 20 health systems across Colorado that participate in WWC to assist women newly diagnosed with breast or cervical cancer. WWC clinic staff can contact COE patient navigators to get help with their patients, or to refer patients that are in need of assistance. COE navigators assist women in finding affordable treatment options. COE patient navigators speak English and Spanish.
Navigation services provided by COE are provided at no cost to patients or WWC clinic sites.
