TCPi Overview and Resources

Transforming Clinical Practice Initiative (TCPi) was a free, nationwide initiative funded by The Centers for Medicare & Medicaid Services (CMS) that aimed to prepare clinicians to be successful in new value based compensation models.

TCPi came to an end September 28, 2019 and we would like to thank you for taking your time and energy to be a part of this work. We appreciate your work to improve the health care system and the quality and experience of care for the people of the State of Colorado. Your dedication to practice transformation and continuous improvement as part of your participation in TCPi demonstrates your leadership. Thank you for taking your time to be a part of this initiative, and we wish you the best in your continued transformation journey.

These resources were developed as part of the TCPi initiative and it is our hope that you can use them for your reference and adapt as needed to fit your needs and that of the practices you support.

TCPi Highlights

  • TCPi impacted 196 practices and 1,973 clinicians practicing in over 25 specialty types
  • Colorado PTN practices improved 100,374 lives during their participation in TCPi
  • This work amounted to avoidance of over 11,500 hospital visits, including 6,017 ER visits, and over 4,000 tests
