House Bill 22-1302 provided the Department of Health Care Policy and Financing (HCPF) funding to support, improve, and expand integrated behavioral health services in primary care settings in Colorado. As part of this program, the Practice Innovation Program provides:

  • practice facilitation support to awarded practices through our CHES Practice Transformation Organization (PTO) partners
  • management and quality assurance of practice transformation
  • subject matter experts to support the behavioral health implementation goals of the awarded practices
