CHES Partners

Practice Innovation Program Colorado coordinates the Colorado Health Extension System (CHES), which convenes numerous Practice Transformation Organizations and Regional Health Connectors across the state for the purpose of achieving better population health, lower costs, and an improved experience of care for patients, families and the healthcare team. Our CHES partner organizations match Practice Facilitators (PFs) and Clinical Health Information Technology Advisors (CHITAs) into practices to support practices in achieving their goals of better health for the population. Together, we guide learning networks that spread best practices and connect participants to local, state, and national transformation resources.

Advocates for Human Potential 

Advocates for Human Potential is a national consulting firm that has a focus on improving behavioral health, expand it’s workforce, and help solve the vexing challenge of homelessness in America. Their work includes addressing the opioid crisis, homelessness and criminal justice.   

Carina Health Network

Colorado Health Providers Alliance (CHPA) and Colorado Community Managed Care Network (CCMCN) are now one organization, uniting data, technology, coaching, and value-based care strategy to support healthcare providers and strengthen systems.

Children's Hospital Colorado Pediatric Mental Health Institute

The Pediatric Mental Health Institute (PMHI) at Children’s Hospital Colorado advocates for and trains on  improved mental health management and works to expand preventative and evidence-based intervention efforts in Colorado. through advocacy, community education, prevention, clinical service development and implementation and research.  

Colorado Access

A local, nonprofit health plan that currently serves members in Region 4 of Colorado (Adams, Arapahoe, Denver, and Douglas counties). For 30 years, Colorado Access has been committed to improving health outcomes and supporting communities in the pursuit of equitable, accessible care for everyone.

 

Colorado Community Health Network
The Colorado Community Health Network (CCHN) is the unified voice for Colorado’s 19 Community – Health Centers (CHCs) and their patients. CHCs provide a healthcare home to more than 650,000 of their community members – one in eight people in Colorado – from 60 of the state’s 64 counties. Without CHCs, hundreds of thoughts of Colorado’s low-income families and individuals would have no regular source of healthcare. CCHN’s mission is to increase access to high-quality healthcare for people in need in Colorado.

Colorado Rural Health Center
The Colorado Rural Health Center (CRHC) is Colorado’s nonprofit Office of Rural Health. The CHRC works with federal, state and local partners to offer services and resources to rural healthcare providers, facilities and communicates. We have a diverse and inclusive statewide constituency of over 3,5000 people and organizations. Our mission is to enhance healthcare services in the state by providing information, education, linkages, tools and energy toward addressing rural health issues. Our vision is to improve healthcare services available in rural communication to ensure that all rural Coloradans have access to comprehensive, affordable, high-quality healthcare.

Comagine Health

Comagine Health is a national, nonprofit, health care consulting firm that works  collaboratively with patients, providers, payers and other stakeholders to reimagine, redesign and implement sustainable improvements in the health care system

Community Care Alliance

The Community Care Alliance (CCA) is an affiliate of Western Healthcare Alliance and serves as a Clinically and Financially Integrated Network (CFIN) for value-based contracting. The CCA is dedicated to partnering with rural providers and hospitals to collaborate on keeping care local, shifting healthcare delivery from volume to value, and improving quality, patient outcomes, and lowering the cost of care.

CONTEXTURE
Contexture—formerly known as CORHIO and QHN in Colorado—is a regional nonprofit dedicated to transforming how healthcare providers access and use digital health records. Contexture's health information exchange (HIE) connects data from thousands of healthcare organizations across Colorado and Arizona to deliver real-time access to critical patient information. This enhances care coordination, improves both patient outcomes and provider experience, and reduces overall healthcare costs. Contexture’s Quality Improvement Team supports healthcare organizations with services like EHR vendor adoption and communication, value-based payment program guidance and clinical quality reporting. Denver Health and Hospital Authority

Denver Health and Hospital Authority (DHHA) is an integrated delivery services system serving the city and county of Denver since 1980. The DHHA community health system collaboration consists of a primary care network of “medical homes” situated with a larger “medical neighborhood” connected organizationally and virtually. Denver Health’s Community Health Services (CHS) is the second oldest and largest community health center program in the United States. CHS has expanded into a network of ten community health centers (PCMH certified), three urgent care centers and 20 school-based health clinics. Denver Health cares for 25% of Denver’s population annually.

High Plains Research Network
The High Plains Research Network (HPRN) is a practice based research network that consists of rural primary care practices, hospitals, health care facilities, and communities located in eastern rural Colorado.  HPRN is committed to providing excellent rural health care by translating the best scientific evidence into everyday clinical practice.

HPRN is part of the  SNOCAP family of practice-based research networks. As such, we conduct research and quality improvement programs that reflect the social setting of primary care practice, including practice organization, patient population, and the community.

John Snow Inc. (JSI)

Long Game Health is a boutique, niche consultancy comprised of real-life, ACTIVE, clinical operators with an interdisciplinary expertise in clinical, financial, operations and technology arenas. Remedy HealthCare Consulting is a clinician-owned and operated small business specializing in project management, quality improvement, and data coordination services for public health and healthcare systems. From transformation with TransforMED, a subdivision of AAFP, to large state initiatives in Maryland, Delaware, and Nebraska, Remedy has worked with a multitude of practices, FQHCs, and Rural health clinics.  In addition, we have two highly skilled team members who know EMR implementation from a small community to large health system.  With over 15 years of experience supporting local, state, and federal agencies—including HHS-aligned initiatives and CDC-funded programs—Remedy offers data-driven, scalable solutions that enhance operational efficiency, stakeholder engagement, and care delivery outcomes. 

Rocky Mountain Health Plans - Practice Transformation Team
The Practice Transformation Team at Rocky Mountain Health Plans (RMHP) partners with primary care practices located on the Western half of the State of Colorado to develop a community of advanced primary care practices by fostering quality improvement at the practice level between physicians and patients with a focus on team-based, patient-centered primary care. A state-of-the-art practice transformation approach is integrated into the medical neighborhood through the implementation of care management and care coordination processes and engagement of both primary and specialty practices

Trinsic

Trinsic is the Clinically Integrated Network owned by UCHealth and Intermountain Health. The CIN includes over 1000 primary care providers across Colorado. We are a network of health care professionals committed to achieving success in value-based programs by working closely with our practices to support each practice's individual journey towards our ultimate goal of high quality, cost efficient, patient centered care.
