Innovation Support Project (ISP) was a support program for family medicine, internal medicine, pediatric, and OB/GYN practices who serve Medicaid patients, funded by the CU Medicine Medicaid Supplemental Funding Program. The program came to an end in 2023 and we would like to thank all who participated for your time and energy to be part of this work.
These resources were developed as part of ISP and it is our hope that you can use them for your reference and adapt as needed to fit your needs and that of the practices you support.
