• The Residency Form must be completed in order for the Registrar’s Office to determine your residency status for BOTH Colorado residents and non-residents
  
• This form is also used by University of Colorado Anschutz to add or update a student’s or applicant’s Social Security number (SSN) or Individual Taxpayer Identification number (ITIN). SSNs are required by various offices on campus for processing financial aid, tuition statements for tax purposes, student employment, selective service verification, and other credential matching. Personal identifiable information is secured and accessed only by school officials with a legitimate educational interest per university policy and federal law. 
• If you chose not to disclose your SSN during the residency verification process AND want to receive federal aid, you will have to work directly with the Office of the Registrar, which may result in a delay in your federal financial aid. 
  

All incoming students must give permission for a background check and students are responsible for any associated costs. The contact is made through PTCAS by an external vendor called Certiphi when PTCAS is notified of your accepted offer by the Program. The student will receive an email from Certiphi/Application Station that includes a verification code.

If you are planning on applying to the CU PT program and have previously been convicted of a misdemeanor related to any type of assault or have been convicted of any type of felony, please contact Dr. Lara Canham, PT, DPT, OCS, Director of PT Admissions, either by e-mail or at 303-724-9144 to discuss your situation confidentially​.

►IMPORTANT: Individuals for whom all transcripts are up-to-date (with degrees posted and ALL coursework completed and posted) in PTCAS will NOT need to provide additional hard copy transcripts as the PT Program will obtain the transcripts directly through the PTCAS system.

For those finishing their final prerequisite(s)/degree during the winter term (for which course grades/degrees are typically posted in March/April), please request updated transcripts to be submitted to PTCAS.

For those finishing their final prerequisite(s)/degree in spring (for which course grades/degrees are typically posted in May/June), note that PTCAS will only upload official transcripts received prior to June 1, 2026. Therefore, official updated transcripts including spring courses need to be submitted directly to the CU PT Program. When possible, electronic transcripts are preferred. Please send electronic transcripts to [email protected] to the attention of Amy Sturrock, Admissions Advisor. If hard-copy official transcripts are required, request that they are sent to the following mailing address: University of Colorado Physical Therapy Program, Attn: Amy Sturrock, MS C244 13121 E. 17th Ave, Room 3122, Aurora, CO 80045. If you anticipate that your final transcript will not be submitted to CU until after the start of summer courses, please reach out to [email protected] to let us know when we can expect your transcript.

CU Physical Therapy uses a vendor to collect and track all immunization requirements for incoming students. You will need to follow the instructions provided below to comply with the immunization requirements by May 19, 2026.

• All required immunization records are collected and tracked by American DataBank/Complio. To learn how to create your account and upload the required immunization records/documents, please download the student manual.
• In addition to the manual, American DataBank/Disa Healthcare has user videos available.

Certification is due by May 19, 2026. Please upload a copy of your certification card into American Databank/Complio as you did for immunization records. You will need copies of both sides (photo images taken with a camera are not accepted) of your BLS/CPR certification card.

All students in the School of Medicine are required to maintain BLS for Healthcare Providers certification. The required course is the Health Professional CPR/BLS course that includes adult, children, infant, and AED competencies. First Aid courses are not necessary and cannot be substituted for CPR. WE WILL ONLY ACCEPT COURSES TAKEN THROUGH THE AMERICAN HEART ASSOCIATION. Go to the American Heart Association website to find a course near you.

When your certification expires, you can renew it on the CU Anschutz campus; courses are available through Campus Student Services and require a fee paid directly by the student.

All incoming students (Residential AND Hybrid): please use this form to submit a photo for badging purposes. This must be done by Wednesday, April 1, 2026. 

The CU Passport Student Portalis your one-stop shop to complete many of your student transactions at the University of Colorado. Through the portal, you can access your student center account, pay tuition and student fees, access course materials through the Canvas learning management system and much more!

How to Access the CU Passport Student Portal

• To access your CU Passport Student Portal you must first activate your CU Passport account
• Visit MyAccount and complete steps 1‐5 *For step 1 you will need your Student Identification number (SID). You should have received an email with your Student ID Number (SID) from the Office of Admissions. 
• Ensure you remember your password
• Upon completion of step 5 you will receive a “step 6” screen that will provide your email address and your CU user name
  
  

Both your email address and user name will be used in conjunction with your password to gain access to the CU Passport Student Portal. 

The University assigns all enrolled students with a [email protected] email account. Faculty and staff will exclusively use your University email to communicate with you. Your university email is provided to you when you activate your CU Passport Student Portal. 

How to access your University email 

• After accepting your loans, follow the instructions in your award notice to complete the Master Promissory Notes and Entrance Counseling.
• The week before your semester begins, aid will disburse directly to your student account to pay tuition and fees first. Excess funds will be issued to student in a refund. (See Bursar's Office about setting up direct deposit to your bank account).

2. In the upper left navigation, click on "Student"
3. Click on "Student Center", then "All Student Functions"
4. Scroll down to "Personal Information" and click on "Emergency Contact"
5. Click on "Add Emergency Contact"
6. Enter the information for your Emergency Contact and hit save to complete the form
   
   

As a reminder, your login is your username or your CU email address and the password you created during the UCDAccess Student Portal setup process.
