Strategies for Securing Institutional Support for Fellowship Participation

How to align your Fellowship in Health Quality Leadership Participation with Organizational Priorities

By: Katie Raffel, MD | June 4, 2026

1. Anchor the Request in Organizational ROI

Frame your Fellowship in Health Quality Leadership participation as an investment with measurable return, not a personal development expense.

Many fellows design and implement Improvement Academy projects that directly impact institutional priorities, including:

  • Cost reduction/value improvement (e.g., reducing inappropriate transfusions, decreasing excess ventilator days)
  • Operational efficiency (e.g., improving infusion center throughput, optimizing APP staffing models)
  • Revenue enhancement (e.g., expanding access to appropriate atrial fibrillation ablation, improving capacity utilization by reducing length of stay for patients with pneumonia)

Suggested approach:

  • Identify a high-priority institutional problem before applying.
  • Explicitly link your fellowship project to financial and operational outcomes.
  • Offer to track and report ROI (cost savings, throughput gains, revenue impact).

2. Position as Part of a Leadership Development Package

For individuals entering or currently in leadership roles, Fellowship participation can be framed as part of a standard leadership development investment.

Health systems commonly fund:

  • Executive coaching
  • Leadership courses or certificate programs
  • External training aligned with system priorities

Suggested approach:

  • Incorporate the fellowship into offer negotiations or annual professional development planning.
  • Position it as a structured, outcomes-oriented alternative to more diffuse leadership spending.
  • Emphasize applicability to current role (not theoretical training).

3. Use a Cost-Sharing (Matching) Model

A practical and often successful strategy is to propose a shared investment model. Several fellows have secured support by:

  • Committing personal CME/professional development funds (e.g., $3,000–$3,500).
  • Requesting the institution to match or partially match that contribution.

Why this works:

  • Demonstrates personal commitment.
  • Reduces perceived financial risk for the organization.
  • Makes approval more feasible within constrained budgets.
