Board-certified, board-eligible, or impending graduates of Internal Medicine, Family Medicine, or Medicine-Pediatrics residency programs are eligible to apply. (At this time, we are only able to accept applicants with U.S. citizenship or permanent residence.)
The application for our fellowship includes:
- A completed CU Diabetes Fellowship online application
- A personal statement (limit one page)
- An updated Curriculum Vitae (CV)
- Three recent (within the past twelve months) letters of reference, including one from your residency program director, uploaded
- Photograph
