Take I-25 to Exit 194 for C-470.

Follow the ramp for C-470 West towards Grand Jct.

Merge onto C-470 West and continue to Exit 18.

Take Exit 18 for Lucent Blvd.

Turn left onto Lucent Blvd.

At the second light, turn right onto Plaza Dr.

Make the next right into the CHCO Driveway.

Park in the lot to the left.

Free parking is available for patients in the parking lot in front of the main hospital entrance.

Enter through the Main Entrance and proceed to the elevators off to the left.

Take the elevator to the 4th floor.

Turn right off the elevator towards the outpatient clinic.

Check-in for your appointment with the outpatient clinic check-in desk.
