R2P2 Use Cases
Overall, the Foundational Projects below took part in designing and refining the tools within this website. The Use Case Projects put the tools to the test. Along the way the R2P2 team makes improvements as feedback is generated from the Use Cases.
Foundational Project 1: My Own Health Report (primary care)
Aim 1: Design and integrate a patient-centered health risk assessment and goal setting tool into the UCHealth Mobile App (patient portal).
Aim 2: Explore two distinct methods of follow up; messaging and health navigation calls.
Foundational Project 2: Carotid Stenosis Decision Aid (specialty care)
Aims: Develop two patient-centered decision aids for carotid stenosis: 1) Symptomatic (stroke TIA) and 2) Asymptomatic (no stroke or TIA).
Foundational Project 3: Heart Transplantation Decision Aid (tertiary care)
Aim 1: Design a patient-centered decision aid supporting heart transplantation.
Aim 2: Pilot test the decision aid.
Foundational Project 4: Dementia Roadmap (late-life care)
Aim 1: Design a patient-centered roadmap for decision making support in the dementia care continuum.
Aim 2: Pilot test the roadmap in two distinct ways: 1) with Patient and Care Partners and 2) in Community Organizations.
Use Case 5: Empowering Frontline Primary Care Vaccine Communication Through a Conversation Guide and Mini-Training for Medical Assistants and Nurses (pediatric vaccines)
Aim 1: Adapt a clinician-tested conversation guide and mini-training for frontline staff, integrating it into real-world clinic workflows through co-design in Colorado primary care clinics
Aim 2: Test the guide in primary care clinics using pragmatic evaluation templates, usability assessments, and time-driven costing
Use Case 6: R2Text (text messaging)
Aim 1: Engage clinical and caregiver partners to co-create/refine the patient-centered R2Text messages.
Aim 2: Pilot test the impact of R2Texts in community-based clinics located in Denver metro area.
