Even if you are familiar with them, explore the content within to find faster ways to actualize these components.
R2P2 Conceptual Model: Rapidity through pragmatic approaches

Aim 1: Design and integrate a patient-centered health risk assessment and goal setting tool into the UCHealth Mobile App (patient portal). Aim 2: Explore two distinct methods of follow up; messaging and health navigation calls.
Aims: Develop two patient-centered decision aids for carotid stenosis: 1) Symptomatic (stroke TIA) and 2) Asymptomatic (no stroke or TIA).
Aim 1: Design a patient-centered decision aid supporting heart transplantation. Aim 2: Pilot test the decision aid.
Aim 1: Design a patient-centered roadmap for decision making support in the dementia care continuum. Aim 2: Pilot test the roadmap in two distinct ways: 1) with Patient and Care Partners and 2) in Community Organizations.
Developing and testing brief, role-relevant, mini-training materials for influenza and HPV vaccines for nurses and medical assistants to promote effective communication and training in pediatric practices.
Aim 1: Engage clinical and caregiver partners to co-create/refine the patient-centered R2Text messages. Aim 2: Pilot test the impact of R2Texts in community-based clinics located in Denver metro area.
