*Applicants applying for the Pediatric Pain Fellowship must email mallory.pridy@cuanschutz.edu directly to indicate their interest in this spot*

Applications for the Pain Medicine Fellowship Program will be accepted through ERAS starting December 1st of every year. The application deadline will be March 1st. Any applications or updates made in ERAS after March 1st will not be considered.

We will require the following documents to be uploaded to your ERAS application:

• CV
• Photo
• Medical School Transcript
• 3 Letters of Recommendation
• Personal Statement
• Board Scores: Either USMLE Step 1, 2, and 3 or COMLEX Level I, II, and III

Please be advised that review of applications will begin in early spring.
