To change your name for CU systems, you must send a copy of your signed Social Security card showing your new, legal name to OME-HR@cuanschutz.edu.
It is important that the GME office and University HR have updated contact information for all residents and fellows.
To update your address:
Log into CU Employee Portal at My.CU.edu with your University Credentials
In the upper left, select My Info and Pay from the dropdown
Click the My Info tile.
IMPORTANT: Update your new address in BOTH the Home and Mailing Address boxes.
Click Save
All residents in GMEC approved programs are required to successfully complete the USMLE Step 3 examination or COMLEX Part 3 examination, as evidenced by obtaining a passing grade for that examination, prior to the mid-point of the second post-graduate year (PGY2).
All fellows entering GMEC approved programs must have successfully completed the USMLE Step 3 examination or COMLEX Part 3, as evidenced by obtaining a passing grade for that examination, prior to starting a fellowship.
Contact your program coordinator, informing them of your need for a new or renewed badge. They will contact the GME Business Services Professional, who will then submit a request to the Security and Badging Office (SBO). As soon as that request is processed you will receive an email from the SBO asking you to contact them directly to schedule a time to pick up your badge.
Residents/Fellows are eligible for the fee exempt DEA because they are paid and are performing service on behalf of the CU Anschutz School of Medicine (state entity). Fee exempt DEAs cannot be used during external moonlighting. For more information please see the DEA Fee Exempt Application Information document.
Please read and follow all steps of the International Residency Rotations Policy prior to sending an resident on an international rotation. Of note, Medical Malpractice and an active medical license in the country of the rotation are REQUIRED.
It is HIGHLY RECCOMENDED that you begin this process at least 6-12 months before the rotation starts.

Standard information regarding your malpractice insurance is listed below:

Policy Number: Since we are a self-trust we do not have a policy number ("self-insured")
Type of coverage (Claims-made or Occurrence): Occurrence-based
Dates effective: First day of training
Date of expiration: Last day of training
Tail Coverage: not needed due to occurrence based coverage

Contacting HR can be done outside of Experian ONLY in the following scenarios –
The individual is requesting verification for housing assistance.
The individual needs assistance with a specific NIH loan forgiveness or similar form.
The individual has been formally offered and accepted a position with CU but has not started yet.
The individual has been separated from CU for more than five years.
The information received doesn’t align with what was provided on an application.
The individual needs a verbal verification of information already received from Experian Verify.
HR can provide start date, end date, position held, and salary with proper authorization. We are unable to provide reference checks or comment on rehire status of former employees.
GME can only verify start and end dates of residency and/or fellowship training. Direct requests to verify start and end dates to GME@cuanschutz.edu.
Training verification requests that require information other than start and end dates must be sent to the appropriate Program Coordinator.
For Dept. Radiology Residency and Fellowship programs, verification requests should be sent to rad.verification@cuanschutz.edu.
For Dept. Family Medicine Residency programs including UCH, Swedish, and Rose should be sent to dfmverifications@ucdenver.edu.

Loan Deferment, Forbearance and Public Service Loan Forgiveness
Public Service Loan Forgiveness (PSLF)
As a University of Colorado Anschutz Medical Campus employee, you may qualify for the federal Public Service Loan Forgiveness (PSLF) Program. CU Anschutz is a qualifying 501(c)(3) not-for-profit employer. Eligible borrowers who enroll in a qualifying repayment plan and make 120 qualifying monthly payments may have the remaining balance of their eligible federal student loans forgiven.
Employer Certification Forms
Employer certification for PSLF is managed centrally by the University of Colorado's PSLF team. The Office of Graduate Medical Education (GME) does not complete or certify PSLF employment certification forms.
If you need an employer certification form completed, have submitted an electronic PSLF request, or have questions about the PSLF process, please contact the University's PSLF team directly at PSLF@cu.edu.

In order to view and/or print a copy of your monthly pay advice you must login to the CU Portal, click on the NavBar (small compass needle in upper right corner of screen), CU Resources, My Info and Pay, then Pay Advice.
Go to the Employee Services - Forms webpage and complete the following forms:

W2/1042-S Reissue Request Form
Fax the forms to 303-860-4299.
To update your W-4 you must login to the CU Portal, click on the NavBar (small compass needle in upper right corner of screen), CU Resources, My Info and Pay, then W-4. You must go through the Two-Factor Authentication to update your W-4
Medical residents are not entitled to the student exception for FICA, according to the IRS Office of Chief Counsel (Tax Exempt and Governmental Entities). The IRS has concluded that generally, unless the facts are virtually identical to those in the Eighth Circuit case (University of Minnesota where residents pay tuition), services provided by medical residents are not incident to and for the purpose of pursuing a course of study, but rather a form of on-the-job training, and therefore not entitled to the statutory exception from FICA.
