PIPCO coordinates the Colorado Health Extension System (CHES), which convenes numerous Practice Transformation Organizations (PTO’s) across the state for the purpose of achieving better population health, lower costs, and an improved experience of care for patients, families and the healthcare team. Click here to learn about the support team that PTO’s can provide.
