We are committed to working with our physician colleagues in Colorado and throughout the nation to provide high-quality patient care. Whether for a second opinion, diagnostics, surgery, treatment, a single consultation or transfer of care, we value the trust you place in us each time you refer a patient and look forward to working with you.
Medical Professionals wishing to refer a patient to our clinic are asked to complete our referral form and fax it to 720-848-5079:

PEDIATRIC PATIENTS:
Children should be referred directly to our faculty at the Eye Clinic at Children's Hospital Colorado: 720-777-2020.
