Being the Change: Innovation and Quality Improvement for Older Adult Primary and Post-Acute Care Settings

March 28 & 29, 2022, Denver, Colorado

Welcome Clinicians, Service Providers and Community Members to the 18th Annual Rocky Mountain Geriatrics Conference. Rising geriatric populations, multiple chronic diseases and burgeoning needs for long term care and home health services are just some of the facets of aging facing our healthcare and service provider communities.  Providers working with older adults need to be equipped with efficient and effective ways to care for this growing patient population. Please join us for 2-days of education and knowledge exchange presented by clinicians and researchers providing the latest information of work being done in the aging sector.  Some of the topics include research in aging information, clinical quality improvement for patients/residents in outpatient, skilled nursing settings, and home health located in urban and rural settings, and new innovation models and frameworks for patient/resident care delivery. This conference provides an unique opportunity to bring clinicians, researchers and service providers, who work with older adults to share best practices, network and learn from each other.

Conference Objectives:

• Describe skills and concepts important for initiating change and prioritizing goals in quality improvement and clinical innovation
• Discover examples of innovative quality improvement initiatives specific to the care of older adults
• Identify unique challenges to quality improvement implementation in the outpatient and skilled nursing clinical environments in both rural and urban settings
• Learn and network with expert geriatric clinicians, researchers and service providers to help solve complex issues facing older adult populations

Key Note Speakers:

• Katherine Jones PT, PhD: Dr. Jones completed her doctoral training through the Medical Sciences Interdepartmental Area program at UNMC with a concentration in health services research. The theme of Dr. Jones’s work is to translate research into structures and processes that improve patient safety and quality of care in rural communities. She was the principal investigator for the CAPTURE Falls project from its inception through her retirement from the University of Nebraska Medical Center (UNMC) DPT Program in 2018. She teaches Team STEPPS through AHRQ and is the President of the Nebraska Coalition for Patient Safety and has many publications related to her work with rural hospitals and critical access hospitals on fall reduction programs and the like.
• Sandeep Wadhwa MD, MBA: Global Chief Medical Officer of 3M HIS. Population health leader with a passion for improving health status of vulnerable populations, particularly those in poverty, people with disabilities, and older adults.
• Read Pierce, MD: Chief, Division of Hospital Medicine, Associate Chair for Faculty Development and Wellbeing at Dell Medical School. Former VP of Culture Transformation and Strategy at The Institute for Healthcare Excellence. Dr. Pierce helped found the Institute for Healthcare Quality, Safety, and Efficiency based at University of Colorado, and led organizational transformation initiatives including programs focused on quality, process improvement methods, culture, teamwork, systems redesign, finance, innovation, and leadership of change.
