GRECC Interdisciplinary Case Conferences (Monthly): A monthly interdisciplinary conference with a focus on communication, interdisciplinary care and team focused care coordination. Case presentations are discussed as a group, with faculty
supervision and input only when needed. Each health profession is encouraged to speak up, and further understand each person’s role in providing comprehensive care for older adults, during patient cases provided.
Journal Club (monthly): Fellows will present once yearly, leading the group in a critical review of current literature related to care of the older adult.
Grand Rounds (twice monthly): Lecture series given by faculty in the Division of Geriatrics and invited speakers with expertise in care of the elderly. Fellows will present once yearly, giving a presentation on their topic of choice.
Systems-Based Practice and Policy Conference (bi-monthly): This seminar aims to meet fellowship milestone requirements and provides an open forum for discussion of healthcare system and policy issues that specifically affect the geriatric population.
Seniors Clinic Case Conference (Monthly): This interdisciplinary case conference held once per month for all Seniors Clinic staff using a representative patient case to focus on clinic processes, interdisciplinary care, and identified systems obstacles.
Morbidity and Mortality Conference (1-2 times per year): Fellows will lead a clinical case discussion of details leading up to and surrounding a poor patient outcome, with focus on system based errors and pathophysiology. This conference should serve as an opportunity to explore errors, biases and prompt discussions on individual practice based improvement.
CMDA Meetings (Monthly): Monthly meeting held by the Colorado Medical Directors Association that provides networking and educational activities for community leaders in long term care and community based services.
Home Based Primary Care Meetings (Tuesday mornings, when available): Interdisciplinary team meeting where home-based-primary care patients are discussed and home care specific education is given.
