Medical Durable Power of Attorney in the UCHealth My Health Connection UCHealth now offers patients the opportunity to complete their Medical Durable Power of Attorney (MDPOA) through the My Health Connection portal. This important document is then uploaded directly into the patient’s medical record and is available to providers within the UCHealth system if ever the need should arise. This work was funded by The Colorado Health Foundation, in partnership with UCHealth.

Advance Care Planning Certification Training Through a Denver Hospice-University of Colorado community-academic partnership, we created and offered training for community members who desire to learn more about advance care planning conversations for themselves and fellow Coloradans (funded by The Colorado Health Foundation).
RELATE is an ideal model of interaction. It includes relationship-centered communication skills designed to be adaptable and transferable to a variety of settings and situations.
