Connective infrastructure links the people, systems and processes that allow healthcare knowledge to move efficiently across Colorado. The Care Collaborative builds relationships, workflows, technology and partnerships that connect:
The result is a stronger, more connected healthcare ecosystem where expertise reaches the point of care, not just the point of origin.
Our mission comes to life through three connected areas of work.
Through ECHO, clinicians participate in collaborative learning communities that build specialty knowledge, confidence and long-term workforce capacity.
Through eConsults, providers receive timely, patient-specific guidance from specialists directly within their clinical workflow, helping patients receive the right care sooner.
Through research, evaluation and statewide partnerships, we develop and refine scalable models that improve healthcare delivery across Colorado.
Collaboration isn't simply how we operate, it's our mission in action.
The Care Collaborative began at the University of Colorado Anschutz School of Medicine, but our work extends far beyond campus. We partner with healthcare organizations, community clinics, public health agencies, researchers, educators and providers across Colorado because meaningful improvements in healthcare happen when people work together.
