Join us February 27-28, 2025 for Bridging the Gap: Seamless Care Transitions from Childhood to Adulthood.
Our two-day conference will focus on transition of care from pediatric to adult care in specialties including urology, cardiology, PM&R, and more; however, any specialty with an interest in transition of care is welcome to attend.

February 28: Join for an online-only gathering with experts in the field to discuss strategies and techniques for managing transition.
