Join us February 27-28, 2025 for Bridging the Gap: Seamless Care Transitions from Childhood to Adulthood.
Our two-day conference will focus on transition of care from pediatric to adult care in specialties including urology, cardiology, PM&R, and more; however, any specialty with an interest in transition of care is welcome to attend.

February 27: Join us for an in-person meet and greet to network with colleagues working in transition of care. If you’re new to transitional care, meet providers in your same field to start building bridges of your own!
February 28: Join for an online-only gathering with experts in the field to discuss strategies and techniques for managing transition.
