The patient resources provided below were compiled by both the Colorado Multidisciplinary Translation Network (CO-MTN) team and The Care Collaborative team at the University of Colorado Anschutz. 

The Care Collaborative provides education to Colorado healthcare professionals ensuring that they have better access to specialized knowledge. 

To provide education to healthcare professionals, The Care Collaborative utilizes eConsults (electronic consultation) and ECHO (Extension for Community Health Outcomes) to deliver both immediate clinical guidance and long-term workforce development.
