The State Network of Colorado Ambulatory Practices & Partners (SNOCAP) is a practice-based research infrastructure that strives to improve communication and assist with joint research projects with shared staff. SNOCAP reaches 280 primary care practices serving diverse populations around Colorado representing a spectrum of organizational settings from large health system owned practices to Federally Qualified Health Centers to fully independent practices. CO-MTN utilizes SNOCAP in order to successfully implement a statewide approach to Long COVID Care Delivery through SNOCAP-identified primary care clinics (PCCs) to participate in the study.
Electronic consultations (eConsult) are part of The Care Collaborative, a University of Colorado Anschutz Medical School resource that ensures that health care providers can communicate electronically with other providers across Colorado to assist one another in providing care to patients. The Care Collaborative has two programs, Project CORE (Coordinating Optimal Refferal Experiences) and ECHO (Extension for Community Health Outcomes). eConsults are part of Project CORE. eConsults are a secure teleheath consultation service that primary care physicians can utilize within CO-MTN to communicate with Long COVID related questions pertaining to a patient. eConsults act as a bridge to multi-disciplinary clinic care augmenting primary care clinic-based care.
The Extension for Community Health Outcomes (ECHO) is part of The Care Collaborative. ECHO is an online practice-based implementation tool utilized by CO-MTN to train primary care clinic providers and clinic staff on Long COVID. ECHO Provides six 45-minute Long COVID modules that focus on what Long COVID is, the diagnosis and management of Long COVID symptoms, and resources providers can recommend to alleviate Long COVID symptom burden. CO-MTN utilizes ECHO within the primary care clinics to improve provider Long COVID awareness, knowledge, and comfort in managing and navigating Long COVID.
CO-MTN is surveying individuals 18+ with Long COVID/symptoms of Long COVID who are seeking care at CO-MTN study clinics about their Long COVID symptoms and the Long COVID care they've received. CO-MTN also partakes in two community engagement efforts: Community of Practice Meetings which consist of multiple stakeholders discussing ways to improve Long COVID care/care delivery, and Patient Advisory Board Meetings which consist of individuals with Long COVID providing feedback on study materials and discussing their Long COVID experiences.
