The Colorado Multidisciplinary Translation Network (CO-MTN) is a tiered Long COVID Care Delivery Pathway that:

  1. Focuses on integrating Long COVID expertise, education, referrals, and care between multidisciplinary care clinics and primary care practices, their teams, and their patients.
  2. Aims to reduce Long COVID symptom burden and severity for patients.
  3. Strives to streamline the care process for underserved and rural Long COVID patients across Colorado.

What Does CO-MTN Do?

  1. Offers primary care clinics training to improve the knowledge, capacity, and exceptional care they provide for Long COVID patients/patients with symptoms that may be Long COVID in Colorado 
  2. Practice facilitation to effectively implement the PCP care in their practice and coordinate with the MDCs
  3. Provides continued Long COVID education and streamlined care for Long COVID which will improve care integration and patient experience

CO-MTN includes:

  1. The State of Colorado-supported Community of Practice comprised of three geographically distributed MDCs
  2. A state-wide practice-based research network representing 280 primary care practices, known as the State Networks of Colorado Ambulatory Practices & Partners (SNOCAP)
  3. The Care Collaborative which includes: a proven tele-education system used to provide health care practitioners specialty training called: Extension for Community Healthcare Outcomes (ECHO) and electronic consultations (eConsults).
