Glucometer (BG Meter) Upload Guide

Get Started
Create a Tidepool Account
If you do not already have a Tidepool account, click here to create a new account.
Follow the prompts to create a new account.
Sharing your anonymized data with Tidepool is optional. If you opt in to sharing your data, the BDC is not involved in any research or analysis that Tidepool uses your data for at this time.
Install the Tidepool Uploader
Once you create an account, install the Tidepool Uploader.
Log in to the Tidepool Uploader
Open the Tidepool Uploader and log in using the username (email) and password you created for your Tidepool account.
Select your devices
You will be prompted to confirm your Time Zone and select the devices your are using.
Select the meter you are uploading from the list and click Done.
Upload your device
Plug in your meter using a USB cable and click Upload next to the device your are uploading.
If prompted, upload all data available on the device.
Allow the upload to complete. If you encounter an error, try an alternate cable and make sure you have the correct device selected.
Verify your data
Log in to the Tidepool web app.
Make sure current data are visible on the overview page.
Share your data
Under the account name, click Share.
Click Share Data.
Select Share with a Clinic and enter the clinic code:
KSBG-7BV8-4VWB
Click Submit Code.
You will be sharing with BDC Telemedicine.
Get Started
Log in to OneTouch Reveal
Go to the OneTouch Reveal web app and log in with the same account used on the phone app.
Verify your data are syncing
Check to make sure the data on the Overview page are current.
Share your data
Click Settings to go to the Settings menu, then click Profile.
Select Add an Office.
Enter the Clinic Code:
QSHRZC
Click Search.
Confirm you are sharing with the Barbara Davis Center and click Yes.
Unable to Upload?
Complete and Return a BG Log.
Download the BG Log template below.
Patient BG Log Book - Template
Complete the Log and Save.
Provide the last 10-14 days of data and save the file locally on your device.
If you have been keeping your own log, save and continue below.
If you have a handwritten log, take photos of the log and save. If your log is more than one sheet, you will need to submit each separately.
Upload your Log.
Click Here to upload your log.
If submitting multiple files, submit one file at a time then restart the submission form.
