Action Plan
Background
As your intervention is successfully implemented, you want to capture your success in three ways: through patient-centered data, financial impact and in the form of stories.
Track your data in a run chart. Consider if a statistical process control chart is needed or beneficial, but note that this requires statistical support. Your run chart should tell a story of your successes in a visual manner. Ideally, the run chart tracks weekly or monthly intervals of process metrics.
This is your opportunity to reconcile your business case. Return to your initial equation and expectation, and track what your data reveals for your financial impact. Ideally, you have exceeded your initial estimate.
You are the storyteller of your work. Creating an impactful presentation to share broadly with stakeholders and to disseminate to your institution or beyond, is critical. Use the presentation template to share your stories.
Consider:
- What is your slap? This is a version of your problem statement, delivered in a powerful way, often with important numbers.
- What is the patient story that drove you to do this work?
- What are your outcomes: the story, the data, the financial impact.
- What do you need to do more of this work? Consider your ask of the stakeholders.
Now that the intervention is implemented and your team is engaged, you need to consider your sustainment plan. The goal is that your work is now embedded in the culture of your clinic, unit, or organization, and odes not require you to manage it as closely, or at all. Consider what steps do need to be managed, and at what frequency?
- What interval do you plan to assess your data, and who will collect it? This is likely no longer weekly/monthly, but quarterly.
- Is there a frontline person who can manage small barriers or problems that arise?
- Can this person also take over celebrating your wins - perhaps at a less frequent interval.
- Delegate this work and ensure this person is aware of their role - this is an opportunity to build their status and expertise - and can be very motivating!
