The Registry includes currently records of people with diabetes (children and adults) seen at the Barbara Davis Center for Diabetes and at Children’s Hospital Colorado (AMC and Colorado Springs campuses). The Registry includes records of patients with type 1 diabetes, type 2 diabetes, cystic fibrosis related diabetes (CFRD), and rarer forms of diabetes, such as post-pancreatitis related diabetes. Our data team is working with Health Data Compass at the University of Colorado to map and receive clinical data for patients, largely adults, with diabetes who receive care throughout the UCHealth system across Colorado. The first data transmission is expected in May 2021. We estimate that UCHealth will be able to provide clinical data for over 100,000 adult patients with type 1, type 2, gestational, and other forms of diabetes mellitus. To make this very large dataset usable, we have begun working with other experts on campus to track patients from childhood to adulthood between institutions and to incorporate a high specificity algorithm to identify diabetes type using various fields from the electronic medical record.

The Biobank serves as a resource for recruitment of individuals into research studies and as a source of data previously collected for research that can be used for additional studies. The Biobank consists of subjects of all ages with type 1 diabetes, type 2 diabetes, pre-diabetes, gestational diabetes, CFRD, and non-diabetic controls identified through the Diabetes Registry or specific research studies located on the AMC who have indicated their interest in participation in future diabetes research. In most instances, study participants (or their parents, for minors) have granted their consent to use their previously collected EHR or research information for assessment of eligibility and contact to recruit in additional research. Access to subjects’ identity is contingent on willingness of the subject to be approached for this purpose. Authorization to release health information for research purposes is obtained at the time that individuals register as patients or consent to participation in a new IRB-approved research. All subjects are informed that the HIPAA authorization can be withdrawn, similar to informed consent. The Living Biobank, can also serve as a central repository of laboratory results, medications and outcomes ascertained by research studies and deposited with subject’s consent to share.

The existing biorepository at BDC currently tracks over 1,400,000 biological samples obtained from >16,000 study participants with T1D and >120,000 non-diabetic participants. The samples mapped to the Diabetes Registry and/or the Living Biobank can be shared with other AMC research projects. Access to the samples will require approval of the PI of the original study. The DRC staff will be responsible for handling of the samples.

This Barbara Davis Center Core Facility is a CLIA certified and CAP accredited laboratory that has been designated as a NIH/NIDDK North America Autoantibody/HLA Core Laboratory. For two decades, it has served major type 1 diabetes clinical trials, e.g. DPT-1, T1DGC, TrialNet, TEDDY, ITN and many others. The laboratory measures autoantibodies to insulin GAD, IA-2, ZnT8, tissue transglutaminase, and 21-hydroxylase using the “gold standard” radiobinding assays and novel electochemiluninescence assays.  HLA Typing service include standard typing of the DQA, DQB, DRB1, as well as HLA A and B polymorphisms.

The Core provides consultation and training to new investigators in the following areas: 1) body composition and mitochondrial function: DEXA, ectopic fat (liver MRI and MRS, visceral and subcutaneous MRI, muscle MRI and MRS), PEA-POD, TOP-POD, BOD-POD;  2) glycemia: continuous glucose monitoring (CGM), HbA1c, overnight glucose stabilization, bedside glucose monitoring; 3) activity and sleep monitoring: actigraphy of sleep and physical activity patterns; 4) insulin sensitivity and pancreatic function: hyperinsuline­mic euglycemic clamp, hyperglycemic clamp, hypoglycemic clamp, mixed meal tolerance test (MMTT), oral glucose tolerance test (OGTT),  IV glucose tolerance test (IVGTT) 4) metabolism: targeted metabolomics, lipidomics and proteomics, microbiome, metabolic cart, stable isotope tracers and mass spectroscopy. Our goal is to help DRC investigators choose the best match with specific study needs and means, to gain understanding how to perform them, and how to analyze and interpret the results. Consultations are available during the pre-award or early post-award period as well as during the data analysis period.
