First Visit Guide:

When you come for your appointments, please ensure you have the following items:

• Driver's License or a valid ID
• Insurance information
• Referral Letter (if required)
• Reports, X-rays, MRIs, CT scans, etc., and any other relevant medical information
• List of medications you are currently taking (if applicable)

To cancel an appointment:

Call the office during business hours and provide at least 1 day's notice to allow us to offer your appointment slot to patients on our waiting list.

Alternatively,

Cancel your appointment through your My Health Connections portal.

We understand that your time is valuable, and we strive to maintain punctuality. However, there may be occasions when emergencies or patients require additional time, resulting in unforeseen scheduling delays. We apologize if this causes you to wait longer than anticipated.
