How do Regional Health Connectors support practices that are participating in transformation efforts?

Practices participating in transformation efforts receive support from practice facilitators (PFs or coaches), clinical health information technology advisors (CHITAs) and Regional Health Connectors. Unlike RHCs, PFs and CHITAs are matched with practices participating in transformation efforts. All three work together to determine how and when to integrate RHC support into a practice improvement plan.


How do RHCs support practices that are not currently participating in transformation efforts?

RHCs can help practices assess the need for practice transformation support and make connections to the appropriate programs, such as Innovation Support Project, the Facilitation Alcohol Screening and Treatment (FAST)Opioid Management, and more.

RHCs also support practices by connecting them to community resources and programs in the surrounding region.
