Innovation Support Project (ISP)

A support program for family medicine, internal medicine, pediatric, and OB/GYN practices who serve Medicaid patients.


Program Design

A 1-year program for family medicine, internal medicine, pediatric, and OB/GYN practices who serve Medicaid patients, that is designed to align with CPC+,  PCMH, and RAE priorities and address the Medicaid Alternative Payment Model participation. 

  • Practice is open to new Medicaid Patients; or,
  • >200 attributed ACC beneficiaries or >$30,000 in historical annual claims paid; or,
  • Medicaid beneficiaries represent 10% or more of their practice panel; or,
  • Practice is in a rural community designated with a RUCA code of 4 or greater (based on practice zip code)

What Will Practices Receive?

  • Practice facilitation support from the practice transformation organization (PTO) of your choice
  • If needed, support from a clinical health information technology advisor (CHITA) will be provided by the Practice Innovation Program @ CU
  • Services of a Regional Health Connector (RHC) to provide community linkages to support patient needs
  • Potential CME, Maintenance of Certification, and/or COPIC points for participation
  • Support to help improve Colorado Medicaid Alternative Payment Model (APM) measures

What's Expected From Practices?

Form a quality improvement (QI) team to work with the practice facilitator. The QI team generally consists of a provider champion, a staff champion, and representatives from the nursing staff, front desk, and other clinicians as appropriate for the practice. The QI team will meet regularly with the practice facilitator to implement changes in the practice.

  • Submit clinical quality measures on a quarterly basis
