NIH R01-NR016209; 7/1/16-4/1/21 ($2,946,919; directs+indirects). The major goal of this study is to determine if an interdisciplinary approach to home health rehabilitation including nutritional supplementation, improves physical function and reduces re-hospitalizations more than usual care. The major goal of this project is to evaluate whether high intensity, front-loading of home health rehabilitation following hospitalization can improve community mobility, quality of life, and reduce caregiver burden in older Veterans.
