When you schedule your appointment, you will receive information about what type of identification you need to bring and how you can prepare for your appointment or procedure. Please plan to arrive 15 to 20 minutes before your appointment to complete clinic and insurance forms.

We strive to make our billing service as direct and professional as our medical services.

You will receive two bills for care received in this practice: one from CU Medicine and another from UCHealth.

If you have questions about the bill you receive from CU Medicine, please call Patient Account Services at 303-724-5066 or 1-800-298-8682.

Different types of insurance require you to pay a deductible or a co-pay when you receive services. You are responsible for paying your health plan's deductible or co-pay at the time we provide service. For your convenience, we accept cash, personal check, MasterCard, Visa, and Discover.

The CU Orthopedics staff is committed to protecting your health information, a right you have detailed in the federal Health Insurance Portability and Accountability Act (HIPAA) of 1996.

To that end, we have a notice of privacy practices that explains the ways we will use your protected health information (PHI). It also explains the ways we will share or disclose PHI about you. In addition, we may make other uses and disclosures that occur as a result of the permitted uses and disclosures described in this Notice.
