Accessing and Connecting Transition Pathways: Employment and Disability Benefits

This presentation provides an overview of benefits, employment pathways, and community support available to students and youth as part of transitioning into adulthood. Participants will learn how benefits planning, education-to-employment transitions, and community-based resources work together to support meaningful employment, financial stability, and overall well-being.

Learning objectives: 

By the end of this presentation, participants will be able to:

  • Identify and differentiate key child and adult benefit programs, including eligibility, access points, and age-related considerations.
  • Explain the role and importance of benefits planning in supporting employment and long-term financial stability.
  • Describe career pathways and employment models, including supported employment, customized employment, individual placement and support.
  • Recognize available transition and community support resources that are available.
