2023 Alumni Award Nomination Form
Nominations Due Sunday, April 30, 2023
Every year the University of Colorado School of Medicine and the Medical Alumni Association bestows awards to distinguished alumni, house staff alumni, and faculty who have gone on to perform outstanding research, fill positions of significance, and/or serve their communities in extraordinary ways.
Award Nomination Process:
1. Each year the Medical Alumni Association Board of Directors solicit names of nominees for these awards.
2. All nominations must be submitted on the award nomination form and supported by a C/V or resume and at least one letter of recommendation. The complete nomination package must be submitted to the Office of Alumni Relations at the CU Anschutz Medical Campus. Note awards can be made posthumously.
3. Nomination Guidelines: No person may nominate him/herself. Public elected officials may be nominated two years after completion of their term of office. All nominations shall be considered confidential.
4. The Medical Alumni Association Board of Directors will convene in June 2023 to select the award recipients. The deliberations of the board will not be released.
5. Awardees are asked to attend the University of Colorado School of Medicine’s Annual Silver & Gold Alumni banquet held in November 2023 in Denver, Colorado.
6. It shall be the responsibility of the nominator to obtain the required C/V or résumé, cover letter, and nominee contact information.
7. Nominations received, and judged to be qualified and complete, will be referred to the Medical Alumni Association Board of Directors. Nominators will be notified when the nomination process is complete.
8. Late or incomplete packages will not be forwarded for committee consideration. A nomination does not guarantee an award.
9. The Humanitarian Award is the only award that can be awarded to School of Medicine and/or house staff alumni, or School of Medicine faculty.
10. Past Silver & Gold Award winners are not eligible to receive the Distinguished Achievement Award, the Richard Krugman Distinguished Service Award, or the Humanitarian Award.
